Job summary

Autumn Lake at Glade Valley in Walkersville, MD is seeking a dedicated Occupational Therapist (OT) to join our rehabilitation team. This full-time role focuses on individualized therapy to improve residents' daily function, safety, and independence.

Responsibilities include evaluating residents, creating plans of care, addressing ADLs, coordinating with COTAs and nursing staff, documenting progress, and educating residents and caregivers on safe daily activities. Maryland OT license required.

Qualifications

• Active Maryland Occupational Therapist license required.
• Graduate of an accredited Occupational Therapy program.
• Strong clinical, communication, and teamwork skills.
• New graduate Occupational Therapists are welcome to apply.

Responsibilities

• Evaluate residents and develop individualized occupational therapy plans of care.
• Provide skilled treatment addressing ADLs, strength, coordination, balance, and functional performance.
• Establish treatment goals and track resident progress.
• Recommend adaptive equipment and techniques to support greater independence.
• Adjust treatment plans based on resident needs and response to therapy.
• Educate residents and caregivers on strategies for safe daily activities.
• Maintain accurate and timely clinical documentation.
• Collaborate with COTAs, nursing staff, and other rehabilitation professionals.
