Job summary

Agape Hospice Care is seeking a passionate PRN On-Call Registered Nurse (RN) to join our hospice team serving Gwinnett, Forsyth, and Dekalb Counties. You will provide skilled nursing care in patients' homes, coordinate with the interdisciplinary team, and support families during challenging times.

The role requires an RN license, CPR certification, and strong interpersonal skills. Prior hospice or home health experience is preferred, with proficiency in computer use and Microsoft Office

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer; Microsoft Office suite experience preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.

Responsibilities

• Administer skilled nursing care for clients in residence across all ages.
• Coordinate care with interdisciplinary team, patient, family, and referring agency.
• Assume responsibility for coordination of care and documentation.
