Job summary

Three Oaks Hospice is seeking a PRN On-Call Registered Nurse (RN) to provide skilled nursing care in patients' homes within Gwinnett, Forsyth, and Dekalb counties. This PRN role supports the interdisciplinary team and coordinates care with families and referring agencies.

You will deliver compassionate, patient-centered care, maintain accurate documentation, and ensure safety and comfort for all ages. Requires RN license, CPR, and valid driver’s license; hospice experience preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ RN experience; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to support patients and families.
• Ability to cope with family emotional stress and respect diverse lifestyles.

Responsibilities

• Administer skilled nursing care in clients' homes across ages.
• Coordinate care with interdisciplinary team, patients, families, and referrals.
• Assist with care coordination and communication between agencies.
