Job summary

Huebner-Creek-Health- is seeking a compassionate Occupational Therapy Assistant (OTA) to join our in-house rehab team on a PRN basis. The OTA will provide person-centered care to help residents improve independence and quality of life under the supervision of an OT.

The role requires an active Texas OTA license and COTA credential, with SNF or long-term care experience preferred. Net Health EDS experience is a plus, and strong teamwork and communication skills are essential for success.

Qualifications

• Active Texas OTA license
• COTA credential required
• SNF/long-term care experience preferred
• Knowledge of occupational therapy treatment practices
• Experience with Net Health EDS preferred

Responsibilities

• Plan and administer OT treatments to residents to relieve pain and restore function
• Provide skilled OT services under supervision of an OT
• Relay service requests to the OT and coordinate care
• Ensure treatments follow the established plan of care
• Provide clinical support to patients, families, and caregivers
• Monitor patient responses and report changes to the OT team
• Complete required documentation timely per policy and regulations
• Attend meetings designated by the Director of Rehab
