Responsibilities

• Delivers occupational therapy services to patients across age ranges from infants to geriatrics as per unit guidelines.
• Develops and implements individualized treatment plans; evaluates patients using multiple data sources.
• Plans and coordinates therapy goals with rehabilitation and nursing teams.
• Provides direct therapy using varied settings and strategies to meet patient needs.
• Documents evaluation results, treatment plans, progress notes and related reports.
• Communicates with patients, families, physicians and interdisciplinary team about status and needs.
