Job summary

Rockwood Manor in Texas is seeking a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis. You will evaluate residents, develop individualized ADL/IADL treatment plans, and collaborate with the interdisciplinary team to promote safety, independence, and quality of life.

Ideal candidates hold an active Texas OT license, have SNF or long-term care experience, and are skilled in Net Health Electronic Documentation System.

Qualifications

• Active Texas OT license required.
• In good standing with regulatory agencies.
• SNF or long-term care experience preferred.
• Knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration, and clinical decision-making skills.

Responsibilities

• Consult with patients to evaluate physical condition and impact on daily occupations.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADLs and IADLs.
• Assess home environments and recommend modifications.
• Recommend adaptive equipment and provide training.
• Ensure treatments follow the established plan of care.
• Monitor patient response and modify interventions with the interdisciplinary team.
• Provide clinical education to patients, families, and caregivers.
• Collaborate with interdisciplinary team to ensure high-quality care.
• Complete documentation thoroughly and timely in line with policies and regulations.
• Comply with all applicable rules and procedures.
• Attend required meetings with the Director of Rehab.
