Responsibilities

• Screens and evaluates patients with cognitive and/or ADL disorders and develops care plans
• Provides rehabilitative, skilled and medically necessary treatment
• Documents progress and outcomes in the electronic medical record
• Supervises and supports assistants and students as needed
• Ensures regulatory compliance and professional ethics
• Participates in care conferences and trainings
• Communicates with patients, families and health team members about progress and plans
