Job summary

Qualifications

• Licensed to practice as an Occupational Therapist in the state(s) of practice.
• NBCOT certification and valid driver's license when applicable.
• BLS for health care providers required or within the first 60 days of employment.
• Experience in post-acute, long-term care or home health settings preferred.

Responsibilities

• Assesses clients/patients/residents and designs occupational therapy treatments.
• Evaluates and develops therapeutic plans to improve functional outcomes.
• Documents evaluation data and progress; maintains required records.
• Collaborates with providers, case managers and families; may supervise assistants.
