Job summary

Richwood Nursing & Rehab is seeking a PRN Occupational Therapist to evaluate and treat residents across the facility. You will communicate with families, physicians, and the health team; maintain documentation; and support the Director of Therapy in ensuring high-quality care.

Responsibilities include evaluating within 24 hours of referral, developing treatment plans, supervising COTA staff, and coordinating discharge planning with families and caregivers.

Qualifications

• Bachelor’s degree in Occupational Therapy.
• Proficient in computer skills including Casamba and Point Click Care.
• Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professional colleagues.

Responsibilities

• Evaluate residents within 24 hours, or 1 business day of physician referral.
• Develop and obtain approval for an effective treatment plan.
• Treat residents per physician treatment plan and document progress.
• Supervise Certified Occupational Therapy Assistant (COTA) and aides.
• Record evaluations, daily treatment notes, weekly progress notes, and discharge documentation.
• Participate in discharge planning and caregiver training for transitions.
