Qualifikationen

• Active New Hampshire OT license required.
• Strong evaluation skills and practical treatment planning.
• Effective communication and accurate, timely documentation.
• Experience in skilled nursing or post-acute settings is helpful; new grads welcome.

Aufgaben

• Assess daily living abilities, cognition, upper-extremity function, and activity tolerance to determine rehabilitation needs.
• Establish personalized goals reflecting residents' priorities and discharge setting.
• Deliver treatment for dressing, bathing, toileting, transfers and meaningful activities.
• Select adaptive equipment and teach strategies for task management.
• Reevaluate performance, adjust plans of care, and oversee COTA treatment.
• Maintain clear clinical documentation and share recommendations with the rehab team.
