Stern Therapy Consultants is seeking a PRN Physical Therapist to join Pleasant Valley in Derry, NH. You will assess residents, design mobility-driven treatment plans, and work within a skilled nursing setting to promote safer daily movement.

The role offers PRN scheduling with an hourly wage of $55–$60, plus growth opportunities and collaboration with a supportive therapy leadership team.

Formación

• Active NH Physical Therapist license.
• Strong clinical assessment and treatment planning skills.
• Clear communication and accurate documentation.
• Post-acute rehab or SNF experience is a plus; new grads welcome.

Responsabilidades

• Complete PT evaluations considering medical history and current function.
• Set goals for walking, balance, transfers, strength, and endurance.
• Provide individualized exercise and functional training.
• Recommend mobility equipment and instruct use.
• Review progress and adjust plans; supervise PTAs.

Stern Therapy Consultants is looking for a PRN Physical Therapist at Pleasant Valley in Derry, NH. Take an active role in residents' recovery by identifying mobility challenges and delivering skilled treatment that supports safer movement throughout their day in a skilled nursing setting.

What You'll Do

• Complete physical therapy evaluations, considering medical history, precautions, prior mobility, and current function.
• Establish treatment goals for walking, balance, transfers, strength, and endurance.
• Provide individualized exercise and functional training suited to residents' abilities and recovery needs.
• Recommend mobility equipment and instruct residents and caregivers in safe use.
• Review progress, modify treatment plans, and provide clinical direction to PTAs.
• Document services and coordinate mobility recommendations with nursing and rehabilitation colleagues, including discharge needs.
