Job summary

Enhance Rehabilitation is seeking an Occupational Therapist to evaluate and treat patients in a skilled nursing facility setting. You will implement individualized plans of care, collaborate with facility staff, and educate patients and families on progress and outcomes.

You will re-evaluate progress, adjust goals, and ensure timely documentation of evaluations, discharge notes, and daily progress records while supervising Occupational Therapy Assistants.

Qualifications

• Licensed OT in Illinois.
• Experience in SNF setting preferred.
• Strong communication and teamwork skills.

Responsibilities

• Evaluate and treat patients following an individualized plan of care.
• Develop plans of care and set measurable functional goals.
• Re-evaluate patient progress and adjust plans as needed.
• Complete timely documentation including evaluations, re-evaluations, discharges, and daily progress notes.

As an Enhance Rehabilitation Occupational Therapist, you will be a part of growing team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
