Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our dynamic team in a skilled nursing facility setting. You will evaluate and treat patients, develop individualized plans of care, and set measurable functional goals.

As an OT, you will re-evaluate progress, revise goals, and maintain accurate documentation while collaborating with facility staff and supervising Occupational Therapy Assistants to ensure excellent patient care and service.

Qualifications

• Licensed Occupational Therapist with SNF experience preferred.
• Ability to evaluate patients and develop measurable goals.
• Strong communication and patient education skills.

Responsibilities

• Evaluate new patients and develop plans of care with measurable goals.
• Treat patients and educate them on injury, progress and outcomes.
• Re-evaluate patient progress and revise plans of care as needed.
• Complete timely documentation including evaluations, re-evaluations, discharges and daily notes.
• Supervise Occupational Therapy Assistants and collaborate with facility staff.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
