Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our dynamic team in a SNF setting. You will evaluate and treat patients, develop plans of care, and set measurable, functional goals aimed at improving daily living activities.

You will also complete timely documentation, including evaluations, re-evaluations, discharges, and daily progress notes, and supervise Occupational Therapy Assistants to ensure high-quality care and patient outcomes.

• Collaborate with facility staff to ensure excellent care for you patients

• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
