Qualifications

• Graduate of an occupational therapy program accredited by the American Occupational Therapy Association.
• Excellent written and verbal communication skills with internal and external customers.
• Pre-employment drug/alcohol screening and background check.

Responsibilities

• Perform timely initial and annual OT assessments and plans of care.
• Conduct home safety evaluations for potential enrollees.
• Update care plans and measure outcomes at least every 90 days.
• Train families, caregivers and staff to enhance mobility and independence.
