Job summary

Franklin Heights Nursing is seeking a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis. You will focus on improving residents’ independence, safety, and quality of life through meaningful daily activities and functional outcomes.

Responsibilities include evaluating patients, developing individualized treatment plans to maximize performance in ADLs/IADLs, recommending adaptive equipment, documenting thoroughly, and collaborating with the interdisciplinary

Qualifications

• Active and valid Texas Occupational Therapy license
• SNF or long-term care experience preferred
• Knowledge of occupational therapy treatment practices
• Experience with Net Health Electronic Documentation System preferred
• Ability to manage patients with varying personalities and needs
• Strong communication, collaboration, and clinical decision-making skills

Responsibilities

• Consult with patients to evaluate their condition and its impact on daily occupations
• Assess and interpret patient evaluations and test results
• Develop individualized treatment plans to maximize ADLs/IADLs
• Assess living environments and recommend modifications
• Provide training on adaptive equipment to support independence
• Deliver treatments per the plan of care and monitor progress
• Collaborate with the interdisciplinary team for high-quality care
• Complete documentation thoroughly and timely in line with policies
• Attend required rehab meetings as designated by leadership
