Job summary

Concept Rehab is seeking a compassionate Occupational Therapist to join our skilled nursing and post-acute care team in Cincinnati, OH. The role offers Full-Time & PRN opportunities to help patients regain independence in daily activities and functional mobility.

You will perform evaluations, develop treatment plans, and deliver ADL/IADL interventions while collaborating with nurses, physicians, and other therapists.

Qualifications

Degree in Occupational Therapy (Bachelor/Master/Doctorate).
Current Ohio OT license in good standing.
Experience in skilled nursing or post-acute rehab preferred; new graduates welcome.

Responsibilities

Perform comprehensive OT evaluations and functional assessments.
Develop individualized treatment plans based on patient goals.
Deliver interventions focusing on ADLs/IADLs.
Address cognitive, perceptual, sensory, and mobility deficits.
Recommend adaptive equipment and home safety strategies.
Educate patients, families, and caregivers to support outcomes.
Collaborate with nursing, physicians, and rehab staff.
Supervise COTAs as appropriate.
Participate in care plan meetings and discharge planning.
Maintain accurate documentation per regulations.
