Job summary

Aventura at West Park in Cincinnati, OH is seeking a compassionate PRN Occupational Therapist to join our in-house rehabilitation team. The OT will evaluate residents, develop treatment plans, and provide skilled therapy in a skilled nursing and long-term care setting.

Collaborate with an interdisciplinary team and document in Net Health and PCC. Flexible PRN hours with opportunities to pick up additional hours as available.

Qualifications

• Current Ohio OT license or eligibility to practice in good standing.
• Graduate of an accredited Occupational Therapy program.
• Experience in skilled nursing or long-term care is preferred but not required.
• Strong communication, organization, and clinical skills.
• Familiarity with Net Health and PCC is preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled occupational therapy services to improve functional independence.
• Collaborate with physicians, nursing staff, therapy team, residents, and families.
• Maintain timely and accurate documentation in Net Health.
• Utilize PCC as part of the interdisciplinary care process.
• Ensure compliance with all state, federal, and company regulations.
