Village Caregiving - Cape Cod is seeking a PRN Registered Nurse to join our home care team. You will visit clients to perform health assessments, document medical history, and develop personalized care plans for caregivers, with travel as needed.

The role offers a flexible schedule and independence, with opportunities to train new caregivers and collaborate with healthcare professionals to ensure high-quality home health care.

Qualifications

• Graduate of an Accredited Diploma, Associate, or Bachelor’s School of Nursing, or Vocational Nursing.
• Current State License as an RN
• Proof of CPR certification
• Reliable transportation and valid driver’s license
• Willingness to travel for home visits

Responsibilities

• Visit patients in their homes for one-on-one visits.
• Complete initial and ongoing assessments to identify needs.
• Develop care plans to guide caregivers for safe, competent care.
• Educate patients and families on home health care strategies and devices.
• Communicate with patients, families, and health professionals.
• Complete nurse assessment documents on a tablet and upload to CRM.
• Maintain timely schedule updates with the director.

Village Caregiving - Cape Cod, MA is looking for a PRN Registered Nurse to join our home care team! This role includes visiting clients to complete health assessments, document medical history, and develop personalized care plans for caregivers. Must be willing to travel.

Responsibilities

• Visit Patients in Their Homes for One-on-one Visits

• Complete Initial and Ongoing Assessments to Identify the Physical and Environmental Needs of Patients

• Assess Patients and Their Environment to Develop a Care Plan in Order for the Caregiver to Have a Blueprint to Know How to Safely and Competently Take Care of Our Patients

• Educate Patients and Their Families on Proper Home Health Care Strategies and Procedures, and Make Recommendations for Devices or Tools That Might Improve the Patient’s Quality of Life

• Provide Effective Communication to Patients/Clients, Their Family Members, Team Members, and Other Health Care Professionals

• Fill outNurse Assessment Documents on a Supplied Tablet the Same Day As Assessment. Upload Assessment to CRM.

• Keep Timely Communication with the Director on Schedule for Upcoming and Current Assessments.

Qualifications
