Job summary

PruittHealth Hospice seeks a PRN Nurse Practitioner to join our hospice and palliative care team delivering compassionate, patient-centered care in home or care facility settings. You will perform adult/geriatric assessments, develop and manage care plans, and collaborate with a multidisciplinary team.

Current RN and NP licensure in state, board certification in adult, family, geriatric, or AG primary care required.

Qualifications

• Current, active RN license in practice state and NP credential.
• Board-certified in adult, family, geriatric, or adult-gerontology primary care.
• Experience in hospice or palliative care is preferred.

Responsibilities

• Proactive, collaborative team member in a hospice care environment.
• Conducts comprehensive adult/geriatric assessments and manages plan of care.
• Maintains timely and accurate documentation.
• Follow policies related to scope of practice and continuity of patient care.

Qualifications

ICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS

• Current, active, and unrestricted Registered Nurse (RN) and Nurse Practioner (NP) licensure in state of practice
• Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care
