Job summary

PruittHealth Hospice is seeking a Nurse Practitioner to join our team delivering compassionate hospice and palliative care in patient homes or care facilities. The NP will assess hospice patients, develop and manage care plans, and coordinate with a dedicated interdisciplinary team.

Qualified candidates hold active RN and NP licensure in the state of practice, and board certification in adult, family, geriatric, or adult-gerontology primary care.

Qualifications

• Current, active, and unrestricted RN and NP licensure in state of practice.
• Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care.

Responsibilities

• Proactive, collaborative team member in a hospice care environment.
• Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care.
• Commitment to caring for patients and partners.
• Complete all documentation and records in a timely and accurate manner.
• Follow established policies and procedures as related to the scope of practice.
• Maintain continuity of patient care in a hospice setting.
