Job summary

Optum Home & Community Care, part of the Optum family, is seeking a PRN NP or PA to join the Senior Community Care (SCC) team in New Jersey.

The role involves a one-time comprehensive health assessment in short stay settings, collaborating with case management to support care coordination, reduce readmissions, and improve documentation and patient outcomes. It is an autonomous, rewarding role with limited hours.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• NCCPA certification and state licensure.
• Active, unrestricted licensure in the practicing state (or ability to obtain by start date).
• Driver’s license and access to reliable transportation.
• Ability to gain a collaborative practice agreement where applicable.
• Ability to navigate varied care environments.

Responsibilities

• Work with primary care physicians to provide the best care possible.
• Collaborate with nursing staff and patients' families.
• Collaborate with case management team.
• Documentation, coding and gap closure.
