Job summary

Medical City Fort Worth is seeking a Social Worker (MSW) to perform non-clinical biopsychosocial assessments, crisis intervention, and discharge planning on inpatient floors and ED. This PRN role covers weekends and weekdays 8:00am–4:30pm, with orientation during weekdays.

You will collaborate with care teams, address discharge barriers, and assist families in navigating financial and resource needs while supporting patient-centered care. Training and ongoing support are provided.

Qualifications

• Master's Degree in Social Work required.
• LCSW or LMSW/RCSW-I license required or preferred.
• Certification in Social Work preferred.
• Licensed Clinical Social Worker preferred.
• Minimum of 1+ years medical social work experience.

Responsibilities

• Work collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff to identify and address patient needs.
• Identify and address barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Maintain knowledge of Medicare, Medicaid, and private payer regulations and managed-care concepts.
• Perform thorough assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate and facilitate meetings between care team and patients/families/caregivers; support patient-centered decision making.
• Intervene for psychosocial needs and provide counseling, advocacy, and referrals.

Education

Master's Degree in Social Work

Licensed Clinical Social Worker (LCSW) / LMSW/RCSW-I

Certification in Social Work

Licensed Clinical Social Worker (preferred)

Medical social work experience (1+ years)
