Job summary

Monarch Communities in Delaware is seeking a Wellness Nurse (LPN) to provide direct nursing care with a holistic focus. You will supervise day-to-day nursing activities performed by Certified Nursing Assistants and report to the Health and Wellness Director.

The position is PRN with hourly pay between $32.00 and $36.00, reflecting community needs and compliant with state regulations. Prior long-term care experience and Alzheimer’s care are preferred.

Qualifications

• LPN license required and current license in good standing.
• At least 2 years’ experience in long-term care, assisted living, home health or hospital setting preferred.
• Experience with residents with Alzheimer’s or dementias is preferred.
• Proficiency with computers and Microsoft Office; ability to learn new apps.

Responsibilities

• Monitor health, safety, and well-being of residents.
• Assist in medication administration training and monitoring.
• Supervise care staff in line with regulations and policies.
• Maintain clinical quality assurance per standards.
• Lead service delivery aligned with the community mission.
• Conduct thorough resident assessments.
