Job summary

Nest & Care, based in Montgomery County, MD, seeks an experienced Licensed Practical Nurse (LPN) for a PRN home health role, conducting client assessments and caregiver training for clients in and around Montgomery County, MD.

The ideal candidate is flexible, compassionate, self-motivated, and enthusiastic about working with elderly clients while mentoring caregivers and providing high-quality in-home care. The agency is nurse-owned and operates 24/7 home care services.

Qualifications

  • Graduate of an accredited school of nursing, or proof of successful challenge of LPN Exam.
  • Minimum of one (1) year of recent home care experience for pediatric, adolescent, and geriatric patients.
  • Current unrestricted LPN license in the State of Maryland
  • Current IV Certificate
  • Current CPR and First Aid certification
  • TB vaccination and Hep B vaccination
  • Graduation from an accredited nursing program, as indicated by transcript or diploma
  • Clear criminal background and driving record
  • Two professional references within the healthcare field
  • Prior home care experience strongly preferred
  • Self-starter with the ability to work independently and manage time effectively
  • Strong interpersonal skills, including willingness to teach and mentor caregivers
  • Comfort with technology; EMR experience preferred
  • Ability to drive and travel to cases as assigned
  • Weekdays and some weekends availability

Responsibilities

  • Assisting in assessing, planning, implementing, and evaluating patient care.
  • Conducting and documenting ongoing data collection related to patient/family reporting to the RN following guidelines, policies, and procedures.
  • Providing care to age-specific populations (children, adolescents, adults or older adults) using the nursing process.
  • Providing direct or indirect patient care services including medication administration.
  • Demonstrating the ability to perform and respond appropriately in emergency situations.
  • Completing nursing care documentation for assigned patients.
  • Developing and maintaining effective communication with patients, families, physicians, colleagues, and co-workers to ensure ongoing patient care.
  • Interacting with patients, families, staff and others in a courteous, honest and compassionate manner.
  • Observing patients, charting and reporting changes in conditions; taking necessary action.
  • Administering prescribed medications or starting IV fluids and recording times/amounts.
  • Answering patients' calls and determining how to assist them.
  • Measuring and recording vital signs (height, weight, temp, BP, pulse, respiration).
  • Providing basic care and treatments (temperature, BP, wound care, bedsores, enemas, douches, alcohol rubs, massages, catheterizations).
  • Assisting with bathing, dressing, personal hygiene, mobility in bed and standing/walking.
  • Working with health care team to assess needs, plan care and implement interventions.
  • Recording food and fluid intake/output.
  • Evaluating nursing intervention outcomes with the health care team.

Nest & Care is currently seeking an experienced Licensed Practical Nurse (LPN) for a PRN position performing home health visits, client assessments, and caregiver training for our clients in and around Montgomery County, MD.

This position is ideal for a Licensed Practical Nurse (LPN) who is passionate about providing exceptional home care to clients. Nest & Care is looking for a flexible, compassionate, caring, and self-motivated individual who is excited about working with elderly clients and mentoring caregivers. Responsibilities
  • Assisting in assessing, planning, implementing, and evaluating patient care;

  • Administer prescribed medications or start intravenous fluids, and note times and amounts on patients' charts;
  • Answer patients' calls and determine how to assist them;
  • Measure and record patients' vital signs, such as height, weight, temperature, blood pressure, pulse, and respiration;
  • Provide basic patient care and treatments, such as taking temperatures and blood pressure, dressing wounds, treating bedsores, giving enemas, douches, alcohol rubs, and massages, or performing catheterizations;
  • Help patients with bathing, dressing, and personal hygiene, moving in bed, and standing and walking;
  • Work as part of a health care team to assess patient needs, plan and modify care and implement interventions;
  • Record food and fluid intake and output;
  • Evaluate nursing intervention outcomes, conferring with other healthcare team members as necessary.

  • Current unrestricted LPN license in the State of Maryland

  • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
  • Clear criminal background and driving record
  • Two professional references within the healthcare field
  • Prior home care experience is strongly preferred
  • Self-starter with the ability to work independently and manage time effectively
  • Strong interpersonal skills, including a willingness to teach and mentor caregivers
  • Comfort with technology; prior experience with electronic medical records preferred
