Job summary

Trinity Health At Home is seeking a Home Health Licensed Practical Nurse (LPN) to deliver skilled nursing care in patients’ homes within Montgomery or Prince George’s County, Maryland. The role emphasizes autonomy, 1:1 patient relationships, and a strong nursing judgment under the plan of care.

Qualified applicants have at least 1 year in acute care, Maryland LPN licensure, CPR, and a valid driver’s license for reliable home visits.

Qualifications

• Minimum of one year experience in an acute care setting.
• Home healthcare experience preferred.
• Graduate from an accredited or state approved school of practical nursing.
• Current Maryland nursing license.
• CPR certification required.
• Must have passed NAPNES or equivalent medication exam before administering medications.
• Must have a valid driver’s license and reliable transportation to work site.

Responsibilities

• Provide skilled nursing care per plan of care at the patient’s home.
• Deliver consistent 1:1 patient care with autonomy and critical nursing judgment.
• Collaborate with supervising RN and physicians to coordinate care.
• Develop meaningful relationships with patients and families to support goals of care.

Minimum Qualifications

• Must have a minimum of one (1) year experience in an acute care setting.
• Home healthcare experience preferred.
