Job summary

Nest & Care is seeking a dedicated PRN Registered Nurse (RN) to provide essential nursing care to patients in their homes. This flexible, as-needed role supports timely assessments, care planning, and coordination with the care team.

Responsibilities include completing initial assessments, developing and updating plans of care, documenting care activities, training caregivers, and ensuring compliance with state and federal laws. Travel to cases is required.

Qualifications

• Must hold an MD state RN license with Trach care experience
• CPR and First Aid certification required
• TB and Hep B vaccination required
• Graduate of an accredited nursing program with transcript or diploma
• Clear criminal background and driving record
• Two healthcare references
• Prior home care experience preferred
• Ability to work independently and manage time effectively
• Strong interpersonal skills and willingness to mentor caregivers
• Comfort with technology and EMR systems
• Willingness to travel to cases as assigned

Responsibilities

• Meets new and prospective clients and performs an initial assessment of service needs.
• Develops a Plan of Care for all new clients and updates as needed
• Communicates with care managers, caregivers, office staff, clients, family members and other medical professionals about client’s needs
• Consults with the Clinical Director regarding supervision of services
• Provides training to clients regarding their care when requested
• Maintains clinical records and documentation in compliance with state and federal laws
• Performs client evaluations every 30, 90 or 120 days as needed by the client’s Plan of Care
• Performs caregiver evaluations every 30, 90 or 120 days as needed by state requirements
• Provides skills evaluations for new staff during orientation
• Trains new staff and existing staff
• Provides Clinical Oversight

• Assures proper maintenance of clinical records and documentation in compliance with State and Federal laws

Qualifications:

• Clear criminal background and driving record
• Two professional references within the healthcare field
• Prior home care experience is strongly preferred
• Self-starter with the ability to work independently and manage time effectively
• Strong interpersonal skills, including a willingness to teach and mentor caregivers
• Comfort with technology; prior experience with electronic medical records preferred
• Ability to drive and travel to cases as assigned
