Job summary

Bluestem Communities PACE program in McPherson, KS is seeking an experienced LPN for PRN shifts to assess, coordinate, monitor, and provide health care services for participants. The role includes skilled nursing, palliative care, in-home services, and supervision support for home care aides within a team-based care model.

The ideal candidate will hold a Kansas nursing license, be at least 18, and have proficiency with Microsoft Office.

Qualifications

• Must be at least eighteen (18) years of age.
• Kansas nursing license from an accredited school of nursing.
• Working knowledge of Microsoft Office Suite.

Responsibilities

• Provide direct patient care in home and clinic based upon policy and procedures.
• Conduct assessments and participate in care planning and nursing care delivery.
• Coordinate in-home services and supervision of home care aides.
• Administer medications and monitor patient status in accordance with care plans.
• Document care accurately in medical records and communicate with the care team.
