Midland Care ConnectionMidland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on \"meeting individual needs through innovative care solutions,\" and we serve people across multiple counties in northeast Kansas.PACE ProgramThe PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.LPN Clinic NurseProvides direct nursing care to participants within the clinic setting as part of an interdisciplinary care team. The LPN monitors participants’ health status by checking vital signs, observing for changes in condition, and reporting findings to the supervising Registered Nurse (RN) or provider to ensure appropriate follow-up. They assist in carrying out individualized care plans, perform basic clinical tasks within their scope of practice, administer medications and treatments as ordered, and document all care accurately in the medical record. The clinic LPN also helps coordinate appointments, supports clinical procedures, and communicates regularly with team members to ensure participants receive comprehensive, well-organized care. Through hands-on nursing support and close collaboration with the care team, the LPN contributes to maintaining participants’ stability, safety, and overall well-being within the clinic environment.HoursMonday - Friday 8:00am - 5:00pmSign On Bonus***$2,500***To be paid at two increments, 6 months and 1 year of employmentRequiredCurrent Licensed Practical Nurse (LPN) license in the state of Kansas, active and in good standingGraduation from an accredited practical nursing programAbility to provide nursing care within LPN scope of practice under appropriate supervisionStrong observation, communication, and documentation skillsAbility to collaborate effectively with an interdisciplinary care teamBasic computer skills and experience with electronic health recordsPreferredExperience working with older adults or individuals with chronic health conditionsPrevious experience in a PACE program, long-term care, home health, or clinic settingFamiliarity with care coordination and person-centered care modelsStrong interpersonal and relationship-building skillsWhat’s in it for you?Health insurance options for employees and dependentsDental, vision, and prescription drug plansFlexible Savings Accounts (FSA) or Health Savings Accounts (HSA), with employer contributionsPaid Time Off (PTO), Personal and Taking Care of You DaysHoliday pay (six holidays per year)403(b) ERISA Retirement Plan, match providedFree group life and AD&D insurance of $20,000On-site education center to provide professional development and CEU creditsFree Employee Assistance Program (EAP) for employees and dependentsPaid monthly volunteer time at non-profit organizationsUniform Allowance ProgramAppy Today! If you are a compassionate and detail-oriented LPN who is passionate about caring for older adults and making a meaningful difference in a clinic-based, team-centered environment, we encourage you to apply.
