Job summary

Midland Care Connection in Kansas is seeking an LPN to join the PACE clinic team. You will monitor patients, administer medications, document care, and coordinate appointments within an interdisciplinary team to ensure comprehensive, high-quality care in a clinic setting.

The role requires active Kansas LPN license, graduation from an accredited program, strong observation and communication skills, and experience with electronic health records.

It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.LPN Clinic NurseProvides direct nursing care to participants within the clinic setting as part of an interdisciplinary care team. The LPN monitors participants’ health status by checking vital signs, observing for changes in condition, and reporting findings to the supervising Registered Nurse (RN) or provider to ensure appropriate follow-up. They assist in carrying out individualized care plans, perform basic clinical tasks within their scope of practice, administer medications and treatments as ordered, and document all care accurately in the medical record. The clinic LPN also helps coordinate appointments, supports clinical procedures, and communicates regularly with team members to ensure participants receive comprehensive, well-organized care.
