Job summary

Bluestem PACE is seeking an experienced Overnight On-Call Night LPN to join our team in McPherson, KS. This full-time role provides comprehensive home nursing care, collaborative care planning, and prompt response to provider communications.

The position requires KS licensure, strong clinical and communication skills, and the ability to work independently during on-call hours from 4:30 pm to 8:00 am, Sunday through Thursday.

Qualifications

• Must be a graduate of an accredited School of Nursing and currently licensed to practice in Kansas.
• Must have a valid Kansas driver’s license and current car insurance.
• Prefer at least 2 years of home care nursing experience, including with frail elderly population.
• Must be at least 18 years old.

Responsibilities

• Administer nursing care during home visits, including wound care, IV hydration and lab draws.
• Collaborate with physicians, families and multidisciplinary teams to plan holistic care.
• Respond promptly to provider communications and document all interactions.
• Deliver emergency response and provide follow-up care.
• Support quality improvement through feedback and staff training.
• Communicate with Clinical Director before the end of shift for morning meeting.
