Job summary

Midland Care Connection in Kansas is seeking an LPN to join the PACE clinic team. You will monitor patients, administer medications, document care, and coordinate appointments within an interdisciplinary team to ensure comprehensive, high-quality care in a clinic setting.

The role requires active Kansas LPN license, graduation from an accredited program, strong observation and communication skills, and experience with electronic health records.

Graduation from an accredited practical nursing program
Strong observation, communication, and documentation skills
Ability to collaborate within an interdisciplinary care team

Responsibilities

Provide direct nursing care within the clinic setting as part of an interdisciplinary care team.
Monitor participants’ health status and vital signs; report changes to RN or provider.
Administer medications and treatments as ordered; document care accurately.
Coordinate appointments and support clinical procedures; communicate with team members.
