Job summary

TruHeart Home Health is seeking a dedicated PRN RN to provide nursing care to patients in their homes across the Bartlesville/Okmulgee area. You will follow a Plan of Care, perform interventions, document status, and educate patients and families.

The role requires an Oklahoma RN license, CPR, a valid driver’s license, auto insurance, and the ability to chart on a computer/iPad. Prior home health or hospital experience is preferred; candidate must be able to work PRN schedules in diverse home

Qualifications

• Graduate of an accredited school of nursing.
• One or more years of experience in home health or hospital setting.
• Current Oklahoma RN license, CPR, driver’s license, and auto insurance.
• Ability to interact with a computer and iPad for charting and documentation.

Responsibilities

• Conduct physical examinations and assessments of patients, including vital signs, physical status, mental status, and diet.
• Develop and implement written Plans of Care with patients following physician's orders and according to agency guidelines.
• Perform and document nursing interventions, including patient education, according to the Plan of Care.
• Demonstrate knowledge and competency in performing baseline nursing skills, including wound care, lab draws, IVs, and more.
• Communicate effectively with patients, families, and healthcare teams.
• Document patient status accurately and concisely.
• Evaluate the effectiveness of nursing services and identify areas for improvement.
• Conform to all applicable Agency policies and procedures.
• Perform related duties as assigned or requested.
• Continuously strive to improve nursing care through in-service education, formal education, and individual research.
• Assume accountability for reporting incidents and complaints according to Agency policy.
