Job summary

Emerald Care Center Midwest is seeking an Licensed Practical Nurse (LPN) to join our Nursing team in Midwest City, OK. The role focuses on delivering compassionate, high-quality patient care with emphasis on safety, teamwork, and timely reporting of changes in condition.

You will administer medications, assist with wound care, monitor residents, and collaborate with an interdisciplinary team to develop individualized care plans.

Qualifications

• Provides direct patient care with focus on safety and comfort.
• Must be licensed as an LPN in good standing in the state.
• Requires ability to perform comprehensive patient assessments.

Responsibilities

• Provide direct patient care to residents, including administration of medications, wound care, and tracheostomy management
• Conduct vital signs, monitor for changes in condition, and report observations to nursing staff
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Participate in care conferences, rounds, and quality improvement initiatives to ensure seamless coordination of care
• Maintain accurate and detailed records of patient care and medication administration
• Collaborate with other healthcare professionals to ensure patient needs are met, and positive outcomes are achieved
