Job summary

HealthMark Rehabilitation is seeking Licensed Physical Therapist Assistants (PTA) for PRN shifts at Crowley Nursing & Rehab in Crowley, TX. The role entails delivering therapy to residents per physician orders, developing care plans, and documenting progress within regulatory guidelines.

Ideal candidates hold a Texas PTA license, demonstrate strong communication, organization, and clinical judgment, and can work within a large interdisciplinary team. Geriatric rehab experience is preferred.

Qualifications

• Texas PTA license/certification required.

Responsibilities

• Provides therapy services per physician orders and standards of practice.
• Develops plan of care with short and long-term goals to support discharge planning.
• Performs skilled treatments according to plan of care.
• Documents observations and patient responses within required timeframes.
• Completes documentation timely per state and federal regulations.
• Handles billing for provided skilled services as required.
• Maintains rehab gym equipment and treatment areas with infection prevention measures.
