Job summary

HealthMark Rehabilitation seeks a Licensed Physical Therapist Assistant (PTA) for PRN coverage at the Crowley Nursing & Rehab facility in Crowley, Texas. The PTA will treat residents and short‑term patients on site, under physician orders and established plans of care.

Qualified candidates hold a Texas PTA license, demonstrate strong communication and organizational skills, and can manage time effectively. Experience in geriatric rehab preferred and a background check is required.

Qualifications

• Must hold Texas license/certification for PTA.
• Must possess strong oral and written communication skills.
• Organizational and analytical skills required.
• Ability to function effectively in multi-faceted systems and to interact with all levels of staff and customers.

Responsibilities

• Provides therapy services in accordance with physician’s orders and accepted professional practices.
• Develops an overall plan of care including treatment, goals and frequency to support discharge planning.
• Performs skilled treatments according to the established plan of care.
• Documents observations and patient response within required timeframes.
• Completes all documentation timely and per state and federal regulations.
• Completes billing for services provided on the date of service.
• Maintains rehab gym equipment and treatment area with infection prevention measures.
