Job summary

Robert J. Dole VA Medical Center in Hutchinson, KS seeks an intermittent Licensed Practical Nurse (LPN) to support Primary Care within the Patient Aligned Care Team (PACT).

You will provide direct patient care, assist with care coordination, and educate patients while documenting and communicating care plans with the nursing team. This position is intermittent with pro-rated pay and no relocation incentives.

Qualifications

• Graduate of a school of practical or vocational nursing approved by the State agency and/or NLNAC accreditation at the time of program completion.
• Active, full, current, unrestricted LPN/LVN licensure in the United States or District of Columbia.
• Six months to one year experience in nursing care settings may be required depending on grade level.

Responsibilities

• Provide direct patient care within Primary Care/PACT in a Community-Based Outpatient Clinic.
• Coordinate patient care and assist with care planning and education.
• Monitor patient status, communicate findings to RN/MD, and participate in huddles and care coordination.
