Job summary

The Veterans Health Administration, Sioux Falls VA Medical Center, seeks an experienced Licensed Practical Nurse to join the Patient Aligned Care Team. You will deliver patient-centered care, reinforce education, assist with procedures, and help ensure efficient clinic flow for Veterans.

As part of an outpatient primary care team, you will perform pre-visit planning, medication reviews, vital signs collection, and coordinate with Registered Nurses and providers to maintain high-quality care.

Qualifications

• Education program must be approved by the appropriate State agency and/or accredited by the Accreditation Commission for Education in Nursing (formerly NLNAC) at the time the program was completed.
• Exceptions include military education accepted for full LPN/LVN licensure with corresponding qualifications.
• Eligibility as a current GS-0620 LPN/LVN employee transitioning under federal statutes.

Responsibilities

• Provide technically proficient, patient-centered nursing care.
• Assist with procedures and administer medications as prescribed.
• Help ensure clinic flow and continuity of care within the Patient Aligned Care Team.
