Job summary

LiveWell Partners is seeking compassionate Licensed Practical Nurses to work one-on-one with patients in their home setting, delivering education and skilled nursing to help patients stay independent and achieve optimal outcomes.

The LPN will collaborate with a team of supportive clinicians; this PRN role offers a flexible schedule (1–2 days per week or 6 visits weekly) with pay-per-visit compensation and bi-weekly payroll, in a patient-centered home care environment.

Qualifications

• 1 year experience in home care, med surg, or ED as an LPN.
• Current LPN license.
• CPR certification and valid driver's license.
• Flexible schedule and ability to work independently.
• Excellent interpersonal skills and reliable transportation.

Responsibilities

• Participate in planning and coordination of total patient care with RN and physician's plan of care.
• Accurately report and record patient condition and care, including changes.
• Prepare clinical/progress notes and summaries.
• Assist patient with activities of daily living and teach self-care techniques.
• Follow universal precautions and emergency procedures during care.

Skills

Summary

We are looking for compassionate Licensed Practical Nurses (LPNs) to work one-on-one with our patients in their home environment to give proper education and skilled nursing to allow our patients to become more independent. The LPN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.

Responsibilities

• Participate in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care, and follow the nursing care plan for each patient.
• Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change.
• Prepare clinical/progress notes and summaries.
• Assist the patient with the activities of daily living while teaching appropriate self-care techniques.
• Follow universal precautions and all emergency procedures whenever giving any aspect of patient care.

Qualifications

• 1 year experience as an LPN in a home care, med surg, or ED setting.
• Current state license as a Licensed Practical Nurse.
• Proof of current CPR certification and valid Driver’s license.
• Ability to work and a flexible schedule.
• Excellent interpersonal skills.
• Active car insurance coverage.
