TruHeart Home Health & Hospice in Norman, OK is seeking a compassionate Registered Nurse to provide prescribed nursing care in patients' homes, following a Plan of Care and physician orders.

The RN will conduct assessments, implement and update care plans, administer medications and IVs, and coordinate with families and physicians to ensure high‑quality patient care across Norman, Edmond, OKC, Choctaw and surrounding areas.

• Education: Graduate of an accredited school of nursing for RN
• Experience: One or more years in home health agency or hospital setting
• Training: Competency assessments must be completed
• License/Certification: Current Oklahoma RN license, CPR, driver's license, and auto insurance

Job Summary:

We are seeking a compassionate and skilled Registered Nurse to join our team at TruHeart Home Health & Hospice in Norman, OK. As a RN, you will provide prescribed nursing care services to patients in their homes, following a Plan of Care (POC) and according to their needs.

Responsibilities:

• Conduct a physical examination of patients, including vital signs, physical assessment, mental status, appetite, and diet, etc.

• Evaluate patients, family members, and homes to determine what health teaching will be required.

• Develop and implement a written Plan of Care with patients, following physician's orders and agency guidelines.

• Perform accurate assessments, develop and implement a written Plan of Care with patients, and adapt these actions as patient needs change.

• Administer medications, treatments, and IVs according to agency policy, accepted standards of care, and home health guidelines.

• Communicate effectively with patients, families, and physicians to ensure quality patient care.

• Document patient status, patient needs, nursing interventions, and patient response.

• Conduct admission, transfer, recertification, and discharge visits for patients.

• Assist with orders, assessments, wound care, lab draws, and other tasks as assigned.

Requirements:

• Education: Graduate of an accredited school of nursing for RN

• Experience: One or more years in home health agency or in hospital setting

• Training: Competency assessments must be completed

• License/Certification: Current Oklahoma RN license, CPR, driver's license, and auto insurance

Necessities:

• Must pass a criminal background check

• Must pass a drug test

• Must have the ability to fill out and turn in a time sheet in a timely manner

• Must respect and take care of company technology placed in your care

Technology/Equipment Operation:

• Must be able to interact with a computer to check email, assess training, and look at your schedule.

• Must be able to interact with an iPad for onsite charting and documentation.

• Instruments and equipment used on a regular basis: thermometer, blood pressure cuff, glucometer, PT/INR machine, PICC line, wound vac, IV skills competency will be evaluated upon hire.

Job Conditions:

• Must have a driver's license and be willing to drive to patient's residence in Norman, OK, OKC, Edmond, and surrounding areas.

• Ability to communicate both verbally and in writing is required.

• Ability to access a patient's home, which may not be routinely wheelchair accessible, is required.

• Physical activities will include walking, sitting, stooping, and standing and minimal to maximum lifting of patients and the turning of patients.

• Environmental conditions: moderate noise level; inherent potential for exposure to bloodborne pathogens, moderate stress, and emotional demands.
