Job summary

Millwood Hospital, a UHS affiliated facility in Arlington, TX, seeks an Assessment Coordinator (RN/Licensed Therapist) to support referral responses and discharge planning from a remote location. The role requires TX licensure and involves coordinating admissions, core measures, and information delivery.

The Access Counselor provides clinical assistance under supervision and completes HBIPS abstraction for admissions. Hours are Monday to Friday, 11 a.m.

Qualifications

• Education: RN or Master\u2019s in a related field.
• Two or more years of intake assessment experience in an acute psychiatric setting.
• Licensed to practice in Texas as a mental health professional (RN, LMSW, LCSW, LPC or LPCI).

Responsibilities

• Inquiry calls are documented and processed per Intake policies.
• Patients are assessed and stabilized prior to leaving hospital or admission.
• Assist with discharge planning and coordinating with on-call physicians.
• Complete admission paperwork and intake logs.
• Conduct follow-up outreach for missed appointments.
• Coordinate pre-certifications with insurance requirements.

Assessment Coordinator (RN/Licensed Therapist)

The Assessment Coordinator responsible for assisting in the process of responding to referrals and other requests for services and information. The Access Counselor provides technical assistance to the discharge planning process, under the direction of the Director of Assessment Referral. This position is also responsible for completing applicable portion of Core Measures (HBIPS) Abstraction Tool for all admissions.

THIS IS A REMOTE POSITION. CANDIDATE MUST HAVE A TX LICENSE.

Work Hours: Monday – Friday (11 a.m. – 7:30 p.m.)

Job Duties / Responsibilities

• Inquiry calls are documented and processed in accordance with Intake policies.


• Patients are assessed and stabilized prior to leaving hospital or admission to hospital arranged.


• On call doctor is notified and assisted with patient interview.


• Administrative approval is obtained for all admissions.


• Admission paperwork is completed in accordance with hospital policy.


• Intake log is completed with all information documented.


• Follow-up calls are made to those failing to show up for appointments.


• Pre-certification is obtained in accordance with insurance requirements.


• Outcome studies forms are completed.


• Guest relations are demonstrated by example and employees in need of guest relations training are counseled.


• Administration is notified of problems or significant events.


• Safety problems are identified and corrective actions taken.


• Medication is obtained after hours as ordered by the physician with proper documentation of drugs removed and accountability maintained.


• Assures that Core Measures Data (HBIPS) Abstraction Tool is completed as it relates to all admissions.


• Other duties as assigned or directed by Director of Intake (Market).

Qualifications

Requirements

• Education: Registered Nurse or Master’s Degree in a human relations or other related field.


• Experience: Two (2) or more years of experience in Intake assessment activities within an acute psychiatric setting. A minimum of two (2) years’ experience in a psychiatric hospital setting including customer contact.


• Licensure: Licensed to practice in the State of Texas as a mental health professional. (RN, LMSW, LCSW, LPC or LPCI)


• Additional Requirements: A working knowledge of the DSM-IV and the Mental Status Exam. A working knowledge of the Addiction and Recovery field. May be required to work flexible schedule and overtime as needed; may be required to work on-call as needed.
