Asheboro Rehabilitation And Healthcare Center in Asheboro, NC, seeks a Physical Therapist PRN to join an excellent in-house rehab team at a long-term care facility.

Qualifications

• Graduate degree in Physical Therapy (DPT/BS+PT)
• Current license/registration in NC

Responsibilities

• Provide a comprehensive PT evaluation based on MD orders.
• Document findings on the evaluation form per guidelines.
• Develop a comprehensive treatment plan with goals, frequency, duration, and modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Provide treatments using modalities and adjust as needed with safety precautions.
• Document daily participation and progress on weekly notes.
• Update treatment plan promptly as indicated.
• Document discharge and in-services on discharge summaries.
• Co-sign notes as needed; maintain professional relationships.
• Complete all required facility documentation forms.
• Document care plans for MDS as required.
• Attend facility and rehab meetings per supervisor/manager.
• Participate in facility committees as indicated.
• Act as rehab delegate to the interdisciplinary team.
• Assist scheduling of patients as indicated.
• Perform screens as required.
• Complete billing and bill patients ethically for PT services.
• Cover other therapists' caseload during absences.
• Supervise junior staff or students; co-sign notes as required.
• Provide in-service education to rehab staff and others.
• Comply with all facility policies and patient confidentiality.
