Job summary

Qualifications

• Active Florida Registered Nurse license
• Current CPR certification
• Valid driver’s license
• Current auto insurance
• Social Security card
• Health statement (last 6 months) stating no communicable diseases
• Ability to meet AHCA/FLClearinghouse Level 2 background screening requirements

Responsibilities

• Conduct initial in-home client assessments
• Complete scheduled reassessments
• Evaluate client needs and changes in condition
• Review and update care information
• Complete required nursing documentation
• Communicate findings with the care team
• Provide nursing oversight as required
• Travel locally throughout the Jacksonville service area
