Job summary

Concierge Care is seeking an experienced Registered Nurse (RN) to conduct in-home assessments and provide nursing oversight across the Jacksonville area, on a PRN field-based schedule. As a PRN RN, you will perform initial assessments, reassessments, update care plans, maintain documentation, and coordinate with the care team while traveling locally in the service area.

Florida RN license and CPR are required, along with a valid driver’s license, auto insurance, and the ability to pass

Qualifications

• Active Florida RN license required.
• Current CPR certification.
• Valid driver's license and auto insurance.
• Social Security card available.
• Health statement within 6 months free of communicable diseases.
• Ability to complete AHCA/FLClearinghouse Level 2 screening.

Responsibilities

• Conducting initial client assessments in the home.
• Completing scheduled reassessments.
• Evaluating client needs and changes in condition.
• Reviewing and updating care information.
• Completing nursing documentation.
• Communicating findings with the care team.
• Providing nursing oversight as required.
