Concierge Care is seeking a Registered Nurse (RN) for in-home assessments across the Jacksonville area. This PRN, field-based role offers flexible hours and home visits rather than a traditional facility schedule.

The RN will conduct initial assessments, perform reassessments, evaluate needs, and maintain accurate nursing documentation while coordinating with the care team to ensure continuity and quality of care.

Qualifications

• Active Florida Registered Nurse license
• Current CPR certification
• Valid driver's license
• Current auto insurance
• Social Security card
• Health statement within last 6 months stating no communicable diseases
• Ability to pass AHCA/FLClearinghouse Level 2 background check

Responsabilités

• Conduct initial client assessments in the home
• Complete scheduled reassessments
• Evaluate client needs and changes in condition
• Review and update care information
• Complete required nursing documentation
• Communicate relevant findings with the care team
• Provide nursing oversight as required
• Help maintain continuity and quality of client care
