Qualifications

Responsibilities

• Evaluates functional status, mobility, endurance, and pain to plan care.
• Develops PT plan of care and conducts interventions to improve well‑being.
• Documents progress notes after each visit and reports outcomes to the care team.
• Teaches patient and family about plan of care and progress toward goals.
• Visits patients per treatment plan and adheres to state guidelines.
