Job summary

Keller Home Care in Alabama is seeking a Physical Therapist to perform patient assessments, develop treatment plans, and deliver home-based physical therapy services. You will evaluate mobility, endurance, and pain, then implement interventions to improve well-being and monitor progress.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and access to reliable vehicle or public transportation.
• Preferred CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates patient’s functional status and PT needs; assists physician in developing the plan of care.
• Provides PT services within scope of practice and plan of care, coordinating with the health care team.
• Observes and reports patient response to treatment to supervising nurse or physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family about plan of care, limitations, and progress toward goals.
• Visits patients per Plan of Treatment; completes progress notes and submits per policy.
• Completes evaluations and develops PT plan of care following state guidelines.
• Reports outcomes of evaluation, goals, and expected frequency of care.
• Functions in diverse home settings regardless of patient background.
