Good Shepherd Hospice in Enid, OK seeks a compassionate PRN Social Worker to serve patients and families in ENID and surrounding communities. The Social Worker provides medical social services in accordance with the interdisciplinary Plan of Care.

Responsibilities include assessing social and emotional factors within 5 days of admission, helping families understand and follow care plans, coordinating resources, documenting, and collaborating with the interdisciplinary team.

Qualificações

• Master’s degree in social work from a CSWE-accredited program.
• Bachelor degree in social work accredited and supervised by an MSW.
• Bachelor degree in psychology, sociology or related field with MSW supervision.
• Must be licensed in the state where care is provided (if required).
• At least one year of social work experience in health care.
• Commitment to the hospice philosophy of care.
• Able to pass background checks and drug screening.

Responsabilidades

• Assist care team by addressing social and emotional factors affecting health.
• Assess patient and family within 5 days of admission to gauge capacity to cope.
• Help patient/family understand and follow medical recommendations and plans.
• Utilize community resources to support patient and family.
• Observe and report social/emotional changes; document in chart.
• Participate in interdisciplinary team planning and care reviews.
• Contribute to ongoing education and committee work as assigned.
