Qualifications

• Licensed Social Worker in Texas (LMSW or LBSW).
• Minimum 1 year of social work experience in healthcare, hospice, or long-term care.
• Hospice experience preferred but not required.

Responsibilities

• Assess the psychosocial needs of hospice patients and their families.
• Provide emotional support to patients and families during end-of-life care.
• Assist families in identifying community resources.
• Collaborate with the RN Case Manager and IDT to develop care plans.
• Participate in IDT meetings to support patient care.
• Assist with bereavement planning and coordinate with the Bereavement Coordinator.
• Document assessments and interventions in the EMR system.

What You’ll Do:

• Assess the psychosocial needs of hospice patients and their families.
• Provide compassionate emotional support to patients and families during end-of-life care.
• Assist families in identifying community resources and financial or social support services.
• Collaborate with the RN Case Manager and interdisciplinary team to develop individualized care plans.
• Participate in interdisciplinary team meetings (IDT) to support patient care.
• Assist with bereavement planning and coordinate with the Bereavement Coordinator as needed.
• Document assessments and interventions in the EMR system
  

What We’re Looking For:

• Licensed Social Worker in Texas (LMSW or LBSW)
• Master’s or Bachelor’s degree in Social Work from an accredited program
• Minimum 1 year of social work experience in healthcare, hospice, or long-term care
• Hospice experience preferred but not required
• Strong communication and counseling skills
• Ability to work independently while collaborating with a care team
• Reliable transportation and willingness to travel locally
