Job summary

Haven Home Health & Hospice in Woodward, OK seeks a compassionate PRN Hospice Medical Social Worker (MSW) to join the interdisciplinary team. You will provide emotional and resource support to patients and families while guiding them through end-of-life planning and bereavement.

This role emphasizes collaboration with nurses, physicians, chaplains, and aides, with weekly IDT meetings and thorough documentation to ensure quality hospice care. The position requires travel within the service area.

Qualifications

• Bachelor's or Master's Degree in Social Work or Sociology from an accredited institution.
• Current Oklahoma Social Worker license or certification, as required.
• One (1) year of experience in hospice, home health, healthcare, or community-based services preferred.
• Excellent communication and interpersonal skills.
• Understanding of hospice philosophy and the end-of-life process.
• Valid Oklahoma driver's license.
• Reliable transportation and current automobile liability insurance.
• Ability to travel throughout the assigned service area.

Responsibilities

• Complete psychosocial assessments and develop individualized plans of care.
• Provide counseling and emotional support to patients, families, and caregivers.
• Assist families with community resources, financial assistance programs, and advance care planning.
• Collaborate with nurses, physicians, chaplains, aides, and the interdisciplinary team to provide holistic patient care.
• Participate in weekly interdisciplinary team (IDT) meetings.
• Maintain timely, accurate documentation in accordance with hospice regulations.
• Support patients and families through grief, loss, and bereavement.
• Assist with discharge planning and community resource referrals as appropriate.
• Represent Haven professionally within the community and uphold our mission of compassionate hospice care.
