Job summary

Ennoble Care in Oklahoma City is seeking an experienced Hospice Social Worker for our local area. The role focuses on psychosocial assessments, care planning, and collaborating with the IDG to ensure high-quality patient-centered care in the home.

The position emphasizes confidential, compassionate support for patients and families, with coordination of services and ongoing documentation across the care team.

Qualifications

• Must be LMSW or LCSW.
• Hospice experience preferred.
• Ability to reliably handle and maintain confidential information.
• Must be a licensed driver with an insured automobile in good working order.
• The ability to communicate well, both verbally and in writing.

Responsibilities

• Perform a psychosocial assessment within 5 days of admission to hospice.
• Participate in IDG meetings and care plan development.
• Observe, record and report changes in patient’s emotional and social factors.
• Maintain and submit written clinical records (initial evaluation, care plan, daily notes).
• Evaluate patient and family response to social work interventions.
• Confirm weekly scheduling of visits with Case Managers to coordinate other staff visits.
• Participate in IDG and QAPI meetings and activities.
• Assist patient/family if declines social work services and monitor evolving needs.
• Participate in discharge planning as needed.
